Our appointment phone number is (805) 682-8153 and we are available to make appointments Monday – Thursday 8:30am to 4pm and Friday 8am – noon.
We are a referral-based office and in most cases will require records from your primary care or other specialist in order for you to be seen. We do our best to triage appointments based on urgency, and notes provided by a referring doctor will help our doctors to understand how soon you need to be seen.
When you make an appointment our staff will send a link via email for you to complete your paperwork online. This is our preferred method for receiving paperwork. If you do not have access to email we will mail paperwork to you or ask that you arrive 30 minutes early to complete the necessary forms.
What to Bring to Your Appointment:
- Current Medication list
- Insurance Cards
- Referral Paperwork
- Relevant tests and films
- Reading glasses if needed
If we do not have your information entered into our system before your appointment we may have to reschedule you for a later date and time.
Most new consultations require approximately 40 to 60 minutes of time. Please ensure that any relevant medical records, including MRIs, CTs and X-Rays are collected before seeing us.
If your imaging was done at Cottage Advanced Imaging or Pueblo Radiology our physicians will be able to access these studies online.
Ongoing Care and Follow-up Appointments
Plans for ongoing care can vary by patient and by problem. Please be aware of the increasing demands being made for our services and the fact that we will need to act as consultants for you and your primary care provider in many instances.
Follow-up appointments generally require 15 to 30 minutes. We strive to be on time for all scheduled appointments. If you are not checked in at our office ten minutes prior to your appointment we may have to reschedule for a later date.
We respectfully request that you provide 24 hours notice for canceled appointments. Appointment reminders are given several days prior to your appointment. We can remind you by text, email or phone – please let us know what is your preferred method of contact. A charge may be assessed for missed appointments.
